1. Tell your employer:
ALL the details of the injury. If you do not know all the details becused you cannot. Make sure allphysical conditions are noted, scrapes, burns, breaks etc. Range of motion, EVERYTHING
2.Tell your health care provider:
ALL the details of the injury. If you do not know all the details becused you cannot. Make sure allphysical conditions are noted, scrapes, burns, breaks etc. Range of motion, EVERYTHING
3. Send your report of Injury form to the WCB
ALL the details of the injury. If you do not know all the details becused you cannot. Make sure allphysical conditions are noted, scrapes, burns, breaks etc. Range of motion, EVERYTHING
But when fill out the form to circle the area injured, place circles on every part of the body that was injured, bruised, scraped, scratched, bumped etc and attach a full sheet on the injuries sustained. Ask you r doctor to help fill out the form. Maybe the injury will not show up for 3 months, but note the location of anything out of the ordinary. DO NOT RUSH THROUGH FILLING OUT THIS FORM.
IF THERE IS NOT ENOUGH SPACE USE AN ADDITION PIECE OF PAPER.
